Medical Forms

After acceptance, you will need the following medical forms. For a full list of needed forms please see the Registration & Tuition Information Page:

• Pennsylvania Physician’s Exam Form
• Pennsylvania Dentist’s Exam Form (Kindergarten & Up)
• Current immunization record

  
28 PA.CODE.CH.23 (School Immunization) requires students in all grades to have the following BEFORE the start of school:

• 4 doses of tetanus, diphtheria and acellular pertussis (usually given as DTP or DTaP)-(1 dose on or after 4th birthday)
• 4 doses of polio (4th dose on or after 4th birthday and at least 6 months after previous dose given)
• 2 doses of measles, mumps, rubella (Usually given as MMR)
• 3 doses of hepatitis B
• 2 doses of varicella (chickenpox)
